  1. Pictures of the front and back of defective lock. Please also include programming sticker if it is an electronic lock. See where to find programming sticker here.
  2. Mailing address: Must be a physical address and NOT P.O. box.
  3. Telephone number.
  4. Brief description of the issue, including approximate age of the lock.
